1) Eye pain (not to be confused with eyelid pain, which is appropriate for a telehealth visit)?

2) Foreign body/Chemical exposure?

3) Recent eye trauma?

4) Visual disturbances (i.e., double vision, partial or complete loss of vision, flashes/floaters, etc.)?

5) Contact lens use?

6) Facial redness?

7) Recent eye surgery?

8) Moderate/severe headache?
